Appointments
Routine eye exams and contact lens checks are seen
on an appointment basis. Time permitting, walk-ins are welcome. For
emergency appointments, such as infections or eye pain, please
call our office, and you will be fit in immediately.
We realize your time is valuable, and we make a commitment
to keep your waiting time as short as possible. If you are
unable to honor your appointment, please call our office as soon
as possible and we will gladly reschedule your visit.
We recommend a complete eye exam yearly for children,
contact lens wearers, and those with certain eye or health conditions.
Parents are advised to have their children’s eyes examined
by age five. Contact lens wearers should also have a six
month check to monitor their eye health. Adults with healthy
eyes and stable eyeglass prescriptions should be seen every eighteen
months to two years.
A full exam can last anywhere from thirty minutes
to an hour. Please bring your most recent pair of glasses, your
contact lenses, and a list of your current medications and allergies
to your initial appointment. Contact lens follow up appointments
take about fifteen minutes, and your contact lenses should be worn
into the office.
Office Hours
We are open 8:30 am to 5:00 pm Monday-Thursday, 8:30
am to 6:00 pm on Friday, and 9:00 am to 12:00 pm on Saturday. If
an emergency occurs after office hours, call our office and listen
to the recording. A pager number will be given for you to
call, and a doctor will be in touch with you as soon as possible.
Insurance
We are participating providers for several insurance
plans. These include VSP (Vision Service Plan), Medicare,
City of Evansville, Alcoa, Blue Cross Blue Shield, Anthem, Medicaid,
and several others. Please provide us with proof of insurance and
any claim forms before services are rendered. Certain insurance
plans cover routine eye exams, and some will only cover medical
office visits such as eye infections. We will be happy to
work with your insurance company so that you can get the most out
of your coverage. Please understand your insurance program
and ask any questions before services are rendered.
Referrals
The success of our practice depends on keeping you
happy, so you can enthusiastically refer your family and friends. Thank
you for your kind referrals.
Payment
Fees should be paid at the time services are rendered
and delivered. In addition to cash and checks, we accept
Mastercard, Visa and Discover. The billing
process and the mailing of statements is very expensive and time
consuming, and this allows us to keep our costs down.
We realize
that no one should be without medical care, and we will make every
effort to help you if you have financial difficulties. Please
speak to our office manager if this is the case, and payment arrangements
can be made. |
